32 year old, hard working software professional presented with refractory migraine
32 year old, hard working software professional presented with "refractory migraine". The surprising part was that the "migraine" never changed sides. On examination ipsilateral (left sided) tenderness was elicited at the junction of neck and occiput. Had a tough time explaining to him that his "migraine" could be actually Cervicogenic Headache. He finally agreed for Left sided Third Occipital Nerve block...and the "relief was miraculous". Radiofrequency lesioning of Third Occipital Nerve and C3 Medial branch was done for achieving long lasting relief. Patient is abundantly thankful now !!
